Understanding the Five Critical Components of the DOT Physical
The Department of Transportation (DOT) physical exam is federally mandated to ensure that commercial motor vehicle (CMV) drivers are medically fit to safely operate their vehicles. It is a comprehensive assessment that goes far beyond a general check-up.
I. Cardiovascular Health: Blood Pressure and Heart Rate
This is one of the most common reasons for temporary disqualification. The exam focuses heavily on hypertension (high blood pressure) because uncontrolled blood pressure significantly increases the risk of stroke, heart attack, and cardiac events while driving.
- Optimal Standard: A driver whose blood pressure is under 140/90 mmHg is eligible for the maximum 2-year certification, provided all other standards are met.
- Stage 1 Hypertension (140-159 / 90-99 mmHg): Certification is typically limited to 1 year, requiring annual monitoring and potential initiation of treatment.
- Stage 2 Hypertension (160-179 / 100-109 mmHg): The examiner may grant a one-time 3-month temporary certificate. This gives you time to start treatment and reduce your blood pressure to an acceptable level.
- Stage 3 Hypertension (≥ 180 / ≥ 110 mmHg): This results in immediate disqualification. You must bring your blood pressure below 140/90 mmHg before a certificate can be issued (usually a 6-month certification follow-up).

- Hearing: You must be able to perceive a forced whisper at a distance of at least 5 feet in the better ear. If you use a hearing aid, the whispered voice must be heard at 5 feet with the aid.

Diabetes controlled by diet or oral medication is usually permitted, but insulin-dependent drivers require specific exemptions and documentation.
