JaRoTi Wellness PLLC (“JaRoTi Wellness,” “we,” “us,” or “our”) is committed to protecting the privacy and security of your health information. This Notice of Privacy Practices describes how your medical information may be used and disclosed, and how you can access this information, in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

JaRoTi Wellness PLLC is required by law to:

- Follow the terms of this notice currently in effect

- Business management activities

For mobile lab services and telehealth:
- Reasonable safeguards are used to protect privacy
- Electronic communications are secured
- Patients are encouraged to ensure a private environment during services

JaRoTi Wellness PLLC uses administrative, physical, and technical safeguards to protect your PHI, including:
- Secure electronic health records
- Limited staff access to PHI
- HIPAA-trained personnel

We reserve the right to revise this Notice at any time. Updated versions will be posted and made available upon request. Changes apply to all PHI we maintain.
