JaRoTi Wellness PLLC (“JaRoTi Wellness,” “we,” “us,” or “our”) is committed to protecting the privacy and security of your health information. This Notice of Privacy Practices describes how your medical information may be used and disclosed, and how you can access this information, in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

1. Our Legal Duty

JaRoTi Wellness PLLC is required by law to:

  • Maintain the privacy of your Protected Health Information (PHI)

  • Provide you with this Notice of Privacy Practices

  • Follow the terms of this notice currently in effect

  • Notify you if a breach occurs that may compromise your information

  • Healthcare services provided to you

  • Payment for healthcare services

This may include medical records, lab results, billing information, and communications related to your care.

  • APRN evaluations and treatment

  • Ordering and reviewing lab work

  • Coordinating mobile lab services

  • Referrals to other healthcare providers

To bill and collect payment for services, including:

  • Insurance claims

  • Billing services

  • Eligibility verification

c. Healthcare Operations

For practice operations such as:

  • Quality improvement

  • Staff training

  • Compliance and audits

  • Business management activities

4. Other Permitted Uses and Disclosures

We may also disclose your PHI:

  • As required by law (e.g., public health reporting)

  • For health oversight activities

  • To prevent serious threats to health or safety

  • For law enforcement purposes when required

  • To business associates who support our operations (with HIPAA agreements in place)

5. Uses and Disclosures Requiring Your Authorization

  • Receive an accounting of disclosures

  • File a complaint if you believe your privacy rights have been violated

We will not retaliate against you for filing a complaint.

7. Mobile Lab & Telehealth Privacy

For mobile lab services and telehealth:

  • Reasonable safeguards are used to protect privacy

  • Electronic communications are secured

  • Patients are encouraged to ensure a private environment during services

8. Safeguards

JaRoTi Wellness PLLC uses administrative, physical, and technical safeguards to protect your PHI, including:

  • Secure electronic health records

  • Limited staff access to PHI

  • HIPAA-trained personnel

9. Changes to This Notice

We reserve the right to revise this Notice at any time. Updated versions will be posted and made available upon request. Changes apply to all PHI we maintain.
