Mobile Lab Services Delivery Policy

JaRoTi Wellness PLLC (“JaRoTi Wellness,” “we,” “us,” or “our”) offers mobile laboratory services to improve patient access, convenience, and continuity of care. This policy outlines the terms and conditions governing mobile lab service delivery.
1. Scope of Mobile Lab Services
JaRoTi Wellness PLLC provides mobile specimen collection services ordered or authorized by a licensed Advanced Practice Registered Nurse (APRN), which may include:
- Blood draws
- Urine, saliva, or swab collections
- Other diagnostic specimens as clinically indicated
Specimen collection is coordinated with approved laboratories and conducted in compliance with applicable federal, state, and local regulations.
2. Eligibility & Scheduling
- Mobile lab services are available to established patients of JaRoTi Wellness PLLC
- All services must be ordered or approved by the APRN
- Appointments are scheduled in advance and subject to availability
- Same-day or emergency services are not guaranteed
3. Service Locations
- Mobile lab services may be provided in a patient’s home or another approved location
- Patients are responsible for ensuring the address provided is accurate
- JaRoTi Wellness PLLC reserves the right to decline service at locations deemed unsafe or inaccessible
4. Patient Responsibilities
Patients receiving mobile lab services agree to:
- Be present at the scheduled time and location
- Provide safe, clean, and appropriate access for specimen collection
- Secure pets and minimize environmental distractions
- Follow all pre-test instructions (e.g., fasting, hydration, medication restrictions)
Failure to meet these requirements may result in cancellation or rescheduling fees.
5. Specimen Collection & Handling
- Specimens are collected by trained and qualified personnel
- All specimens are labeled, handled, stored, and transported in accordance with laboratory and regulatory standards
- Chain-of-custody procedures are followed when required
Once specimens are collected, they are transferred to the designated laboratory for testing.
6. Test Results & Follow-Up
- Laboratory testing is performed by third-party laboratories
- Results are reviewed and interpreted by the APRN
- Patients will be notified of results within a reasonable timeframe
- Abnormal results may require follow-up appointments or referrals
Mobile lab services alone do not constitute diagnosis or treatment without provider review.
7. Cancellations, No-Shows & Fees
- Cancellations must be made within the required notice period of 24 hrs
- Late cancellations or no-shows may result in a fee of $75
- Missed appointments due to patient unavailability are non-refundable
8. Privacy & Confidentiality
- All mobile lab services comply with HIPAA regulations
- Reasonable steps are taken to protect patient privacy during in-home services
- Patient information and specimens are handled confidentially at all times
9. Limitations & Emergencies
- Mobile lab services are not emergency services
- JaRoTi Wellness PLLC does not provide urgent or emergency medical care
- Patients experiencing urgent or life-threatening symptoms should call 911 or seek emergency care
10. Service
- Mobile lab services are limited to designated service areas
- Availability may vary based on staffing, location, and regulatory requirements
- JaRoTi Wellness PLLC reserves the right to modify service areas at any time
11. Right to Refuse or Discontinue Services
JaRoTi Wellness PLLC reserves the right to refuse or discontinue mobile lab services if:
- Safety concerns arise
- The patient environment is unsuitable
- Payment obligations are unmet
- Services fall outside clinical scope or regulatory limits
12. Changes to This Policy
JaRoTi Wellness PLLC may update this policy at any time. Updates become effective upon posting.
