Medical Terms & Conditions (APRN-Led Care)

JaRoTi Wellness PLLC (“JaRoTi Wellness,” “we,” “us,” or “our”) is an Advanced Practice Registered Nurse (APRN)–led medical practice. These Medical Terms & Conditions govern the care and services provided to patients (“you” or “patient”). By scheduling an appointment or receiving services, you acknowledge and agree to these terms.

1. Nature of APRN-Led Care

Care at JaRoTi Wellness PLLC is provided primarily by a licensed Advanced Practice Registered Nurse (APRN) practicing within the full scope of nursing practice as authorized by state and federal law.

APRN services may include, but are not limited to:

  • Health assessments and evaluations

  • Diagnosis and management of medical conditions

  • Preventive and wellness care

  • Ordering and interpreting diagnostic tests

  • Prescribing medications and treatments

  • Patient education, counseling, and care coordination

When required by law or clinical need, the APRN may collaborate or consult with other healthcare professionals.

2. Scope of Practice & Collaboration

  • The APRN practices independently or in collaboration as required by state law.

  • Some conditions or treatments may require referral to another provider or specialist.

  • JaRoTi Wellness PLLC reserves the right to refer patients when care needs exceed the APRN’s scope of practice.

3. Patient Responsibilities

Patients agree to:

  • Provide complete and accurate medical, medication, and health history

  • Inform the APRN of any changes in health status

  • Follow agreed-upon treatment plans

  • Communicate questions or concerns regarding care

  • Treat staff with respect and professionalism

Incomplete or inaccurate information may affect care quality and safety.

4. No Guarantee of Outcomes

Medical care is individualized. JaRoTi Wellness PLLC does not guarantee outcomes, cures, or results. Responses to treatment vary based on patient-specific factors.

5. Telehealth Services (If Applicable)

If telehealth services are offered:

  • Care is provided by an APRN via secure electronic communication

  • Telehealth has inherent limitations and may not be appropriate for all conditions

  • Patients must ensure privacy and a secure environment during visits

  • Telehealth is not appropriate for emergencies

For emergencies, call 911 or go to the nearest emergency department.

6. Not an Emergency or Urgent Care Practice

JaRoTi Wellness PLLC does not provide emergency or urgent care services. Patients experiencing urgent or life-threatening symptoms should seek immediate emergency care.

7. Prescriptions & Medication Management

  • Medications are prescribed only when clinically appropriate and within APRN authority

  • Controlled substances are prescribed in accordance with state and federal regulations

  • Refill requests may require advance notice and/or a follow-up visit

  • Lost or stolen prescriptions may not be replaced

8. Payment & Financial Responsibility

  • Payment is due at the time services are rendered unless prior arrangements are made

  • Patients are responsible for fees not covered by insurance

  • Fees may apply for missed appointments, late cancellations, or returned payments

9. Appointments, Cancellations & No-Shows

  • Appointments must be canceled or rescheduled within the required notice period

  • No-shows or late cancellations may result in fees

  • Repeated missed appointments may lead to dismissal from the practice

10. Privacy & HIPAA Compliance

JaRoTi Wellness PLLC complies with HIPAA and protects patient privacy.

  • Health information is used and disclosed only as permitted by law

  • Patients receive a separate Notice of Privacy Practices

11. Termination of the APRN-Patient Relationship

JaRoTi Wellness PLLC may terminate care with appropriate notice for reasons including:

  • Noncompliance with treatment recommendations

  • Abusive or inappropriate behavior

  • Repeated missed appointments

  • Nonpayment of fees

  • Clinical needs beyond the practice scope

Patients may also discontinue care at any time.

12. Limitation of Liability

To the fullest extent permitted by law, JaRoTi Wellness PLLC is not liable for indirect or consequential damages related to care provided, except where prohibited by law.

13. Changes to These Terms

JaRoTi Wellness PLLC reserves the right to update these Terms & Conditions at any time. Updates become effective upon posting.

14. Governing Law

These Terms & Conditions are governed by the laws of the state in which the APRN is licensed and JaRoTi Wellness PLLC operates.
